Voluntary Long-Term Disability Insurance

Tax Advantage Premium Plan (TAPP)
The Tax Advantage Premium Plan (TAPP) allows employees to have required health plan premiums withheld from their paychecks on a pre-tax basis. Any such premiums for the CSU sponsored coverage you have selected will be deducted each month from your salary before federal and state income and Social Security/Medicare taxes are calculated and deducted.
Eligibility
All CSU employees are eligible to participate in TAPP if they are active CSU employees enrolled in a CalPERS health plan and have health care contributions withheld from their paychecks.
Enrollment
Eligible employees are automatically enrolled in the program unless non-participation is requested at the time of enrollment. There is a $2.04 service charge per year, which is automatically collected by the State Controller's Office through a payroll deduction.

CalPERS Death Benefit
The benefits payable depend on a number of factors such as the member's age, years of service, eligible beneficiary, and whether or not the employee was eligible to retire at the time of death (age 50 with at least five years of service credit).
- To report a death, please contact CalPERS directly at 1-888-225-7377.
- CalPERS will send information and the necessary forms shortly after a death is reported.
- The information on the completed forms serves as the formal application to receive payable death or survivor benefits and to identify family members who may be legally entitled to benefits.
- If you would like to update your beneficiary designation, please complete the Pre-Retirement Lump Sum Beneficiary Designation Form and mail to the address on the bottom of page 2 on the form.

Social Security Survivor Benefits
When an individual who has worked and paid Social Security taxes dies, certain members of the family may be eligible for survivors' benefits.
If eligible, Social Security Survivor Benefits can be paid to:
- A widow/widower - full benefits at full retirement age (currently age 65) or reduced benefits as early as age 60.
- A disabled widow/widower (as early as age 50).
- A widow/widower at any age, if he or she takes care of the deceased's child who is under age 16 or disabled, and receiving Social Security benefits.
- Unmarried children under 18 or up to age 19, if they are attending high school full time.
- Under certain circumstances, benefits can be paid to stepchildren, grandchildren or adopted children.
- Children at any age who were disabled before age 22 and remain disabled.
- Dependent parents age 62 or older.

Lactation Room
CSUSB and the Division of Student Affairs are committed to supporting students and employees who need to express milk or nurse their child during hours of operation by providing lactation rooms in close proximity to their classes or workspaces. These spaces are private, secure, and located throughout the campus. Please note some spaces do require reservation, and mothers expressing milk are expected to make their own arrangements for refrigeration.
