Step by Step Process for Organizations Seeking to Offer In-Service (Optional) Credit
Step 1 - Download and complete the Optional Credit Course Proposal Form

Please complete this optional credit course proposal form with all required information. Ensure all necessary course proposal form fields are filled in. As a reminder, 1 unit of optional credit is based on 15 hours of instruction.
Step 2 - Download and complete the Instruction Information Form

This fillable form must be completed for each instructor listed on the Course Proposal Form. Be sure to include your signature on the blank field located at the bottom of the document. Download, fill in, and submit all forms together for seamless processing.
Step 3 - Submit the completed Instructor Information and Optional Credit Course Proposal Forms to the In-Service Coordinator
Once you have filled in the blank fields on both the Instructor Information and Optional Credit Course Proposal documents, please email the forms to the In-Service (Optional) credit coordinator listed below. The In-Service (Optional) credit coordinator will keep you updated throughout the entire optional credit approval process.
