Miscellaneous Request Form

The Miscellaneous Request Form should be used for the following items:

• AB386 Exception request
• A change to the assigned final exam day/time
• Exception to the unit standard
• International Partnership Program
• Request a new department code
• Request a new department/school name
• Request a new program type
• Request a new subject code
