Affirmed First Name Update Form

CSUSB recognizes that members of our campus community may use affirmed first names other than their legal first name to identify themselves. Affirmed first names may be used as long as its use is not for the purposes of misrepresentation to avoid legal obligations. Affirmed first names will be allowed whenever possible by the university.
